No, I haven't - didn't know I could. However, that would just be masking something that is amiss elsewhere (see below).Have you tried disabling it in the database?
Do you use an extension that alters the .htaccess?
I have seemed to isolate it (partly through brute force and ignorance, and your comment about an htaccess-altering extension) to JCH Optimize. Not sure how it's altered any htaccess files - I don't ever remember asking it to - but the sites that are misbehaving all have htaccess files that have been altered by JCH. That's why changing files fixed it. Not sure what was in the offending files that caused the issue as they have not till the J! 5.2 upgrade.
At least I have a handle on this issues. However, this EXACT same issue occurs on every new install of Joomla now. No extensions added, but, after the install process, I choose "Administrator" and I get this same issue. For that, I close the window and reopen and it's OK.
